What is Prepare for Your Care?
Prepare for Your Care is a step-by-step program that guides you through the process of advance care planning, including:
- Talking with family, friends, and doctors about your personal goals and what matters most to you.
- Naming a trusted person - also called a health care proxy or medical decision maker - who can speak for you in a medical emergency.
- Recording your wishes and decision maker in a legal document like a living will or an advance directive.
How does Prepare work?
Advance care planning with Prepare for Your Care can happen in two ways.
Work 1:1 with a Thyme Care Clinician
Through a series of guided conversations, you, your loved ones, and your Thyme Care Clinician will discuss your wishes and develop your plan. There are videos to watch and information to review and read to help you understand. In each conversation, you’ll learn more about the advance care planning process so you can feel comfortable and make the right decisions for you.
Your Clinician will help you complete your advance directive form. By the last step, you’ll have a plan in writing that you can share with your family, friends, and healthcare team.
Review materials on your own
You can review the materials below and discuss them with your loved ones. We recommend you follow these steps:
- Review the information under “First Conversations” below with your loved ones. Read the appropriate document and watch the videos.
- Download the Question Guide and watch the video under “Follow-up conversations.”
- If you decide to complete an advance directive, call or text your Care Team. They can send you the appropriate forms for your state.
Advance care planning gives you peace of mind, knowing your voice will always be heard. With Prepare, you’ll have the guidance, support, and tools you need to create a clear plan that honors your values and helps your loved ones and care team know what matters most to you.
